Applying is as simple as visiting OSS for an intake packet. You may also find the necessary forms online.

Forms needed to apply for OSS can be found under the forms link. The verification of disability form needs to be completed and signed by your doctor. Forms such as the (application, OSS intake form and consent of release of information form) all need to be completed and signed by the student. If you were in special classes K-12, a copy of your IEP and Psycho Education Evaluation will be needed. In order to expedite the intake process, bring all of your paperwork to OSS as soon as it is completed.
