Identity Theft Verification

Identity theft can occur when someone uses your personal information without your knowledge or permission, including your name, Social Security number, address, or student records. At Barstow Community College (BCC), we take these concerns seriously and provide a formal process to help protect your identity and your academic record.

Why This Process Is Important

  • Investigate suspected identity misuse

  • Correct or secure your academic record

  • Protect your account from fraudulent activity

  • Respond to requests from third parties that may involve disputed records

  • Comply with federal reporting requirements

This verification is required to proceed with identity theft-related claims and protections.

How to Complete the Process

Step 1 — Complete the Identity Theft Verification Affidavit

Fill out every section of the affidavit clearly and accurately. Be prepared to explain:

  • How you discovered the fraudulent activity

  • When you first became aware of the issue

  • Why you believe the activity was unauthorized

Incomplete information may delay processing.

Step 2 — Attach Required Documentation

You must attach copies of the following documents:

  • Valid government-issued photo ID (Driver’s License, State ID, or Passport)

  • Proof of residency at the time of the activity (lease agreement, utility bill, bank or insurance statement)

  • Copy of a police report filed with law enforcement documenting the identity theft

All documents must be clear, legible, and dated.

Step 3 — Submit Within 30 Business Days

Submit the completed affidavit and all required documentation within 30 business days of discovering or being notified of the identity theft.

What Happens After Submission

  • Admissions & Records will review the affidavit and documentation

  • Additional information may be requested if necessary

  • A determination will be made regarding your case

  • You will be notified of the outcome and next steps

Important Information About Identity Theft Claims

By submitting this affidavit and supporting documents, you affirm that:

  • You did not authorize the use of your identity for the activity in question

  • You did not benefit from the fraudulent activity

  • All statements are true and accurate to the best of your knowledge

Knowingly providing false information may subject you to federal penalties.

Protecting Your Information

  • Never share your student login credentials

  • Update your password regularly

  • Monitor your student account and financial aid activity

  • Report suspicious activity immediately

BCC will never request passwords or payment via unofficial email or forms.
